Delete (don't just deprecate) codes in 2.01

In 2.01 various codes are being replaced with numeric codes:

The current proposal is to deprecate the old codes. I propose that we instead remove the old codes.

Removing the codes will make it easier for data consumers to interpret IATI data that is published to version 2.01. They would be able to know the new codes should be in use just by looking at the version number. If all the data a data consumer needs is being published to version 2.01 or later, they would be able to know they can ignore the old codes.

Not deprecating will make it more difficult for publishers to update 2.01 - but the purpose of the integer upgrade is to introduce breaking changes, and publishers do not need to adopt it straight away. Additionally, some of the other changes proposed for 2.01, particularly the proposal to change the structure of all human readable text fields are of a similar level of difficulty for publishers. Thus, if we do delete the codes, it is likely to be this particular change that would hold back publishers.
